Fifth Universal Definition of Myocardial Infarction (2026)
The Fifth Universal Definition of Myocardial Infarction (UDMI) is a joint statement from the European Society of Cardiology (ESC), the American College of Cardiology (ACC), the American Heart Association (AHA), and the World Heart Federation (WHF). Myocardial infarction is now classified into one of three clinical types - primary, secondary, or procedure-related - to reflect underlying pathophysiology, align with the clinical evaluation of patients, and to enable consistent application in practice. The definition incorporates sex-specific cardiac troponin thresholds and encourages the use of imaging to define the aetiology of myocardial infarction with dedicated ICD-11 codes to support adoption
