Heart failure is accompanied by profound metabolic remodeling, but distinct biochemical signatures in HFpEF and HFrEF remain poorly characterised.

In this study, large-scale metabolomic profiling identified phenotype-specific metabolic networks associated with morbidity and mortality. Nitric oxide signaling and purine metabolism emerged as key prognostic pathways in HFpEF, whereas acylcarnitine and amino acid metabolism were strongly linked to outcomes in HFrEF.
