ESC Guidelines Licensing for AI, LLMs, and CDS Tools

ESC offers a dedicated licensing framework for technology teams building AI models, clinical decision support systems (CDS), or digital health platforms that incorporate ESC Guidelines and Scientific Documents.

What You Can License

You can request permission to:

  • Use ESC content (full text, sections, or extracted data) to train generative AI models or large language models (LLMs).
  • Integrate ESC recommendations into AI-driven CDS tools or Electronic Health Record (EHR) systems for real-time clinical support.

Who Manages Licensing

  • For reproduction or translation not related to AI or CDS, licensing is handled by Oxford University Press (OUP). → Request via OUP

Legal Context

Under EU Directive 2019/790, non-commercial scientific research may be exempt from licensing. However, ESC has opted out of unrestricted reuse, meaning explicit permission is required for AI-related applications—even if the content is publicly accessible.

Licensing Principles

  • Scientific research use may be free for eligible institutions and individuals.
  • Operational or commercial deployment—whether internal or external—requires a license.
  • ESC uses an honor-based declaration system. Misuse or false declarations may trigger retroactive fees and penalties.

Integration Disclaimer

ESC authorises the statement: “This product integrates ESC Guidelines.”

ESC requires the following disclaimer: “The European Society of Cardiology (ESC) disclaims all responsibility for the interpretation of the ESC Guidelines recommendations as integrated and presented in XXX and is not responsible to ensure that recommendations included in XXX have been updated with any new published version of the ESC Guidelines.”
