Through active development in pharmacology in cardiology new therapeutic agents have emerged for treatment of decompensated heart failure. Levosimendan can be called as standard inodilator. Myosin activators are new agents under development. Inodilators act primarily as vasodilators, decreasing pulmonary wedge pressure, but also increase contractility. There is also evidence on antistunning effects especially by levosimendan.
Inodilators are energy efficient agents. Early data on the new class of agents called myosin activators have prelimarily also been shown to be neutral in oxygen consumption.
Adverse events are seen especially in new onset heart failure and especially related to hypotension.
Recent data in postoperative patients with heart failure has also been positive measured as in need for postopretive intensive care.
Teaching objective of the lecture:
Perspective on clinical efficacy of inodilators and myosin activators.
Take home messages:
- Hemodynamic effects of inodilators are impressive
- Careful monitoring is needed
