Should heart failure patients be followed up in primary care or in hospitals? Tiny Jaarsma presented the advantages and disadvantages of both structures in different patient cases.

Is follow up in primary care as effective as in heart failure clinics? What about patient compliance? Home-care needs to be considered in heart failure patients. However models of primary care monitoring must be adapted to the existing healthcare structures in each country which, in turn, depend very much on economics and on culture.
