Several models of disease management in heart failure have proven to be successful in specific settings, however there is still a lot of discussion on the optimal model.

During the last decade several randomized clinical trials have been published on successful disease management but there is a large variability in the application of these programs across the world, depending on national recourses and capabilities. In most disease management programs a multidisciplinary approach is used with an important role for heart failure nurses and cardiologists. Key components often include:

  • assessment and intervention of risks and comorbidity,
  • optimized medical management,
  • patient education and self care management,
  • follow-up,
  • access to healthcare,
  • patient involvement and psychosocial support.

It currently is a challenge how to organize patient care for heart failure patients optimally and efficiently.
