Adult Transesophageal Echocardiography (TOE)

The successful completion of the EACVI/EACTA certification provides the formal and European recognition for individuals, governed by a scientific society.

The goals of certification are to protect patients from undergoing transesophageal echocardiographic examinations performed by unqualified persons and to set a European standard for competency and excellence in this field.

What are your benefits from getting the EACVI certification in TOE?

  • Certification in TOE gives credibility and professional legitimacy to an individual by demonstrating his/her competency on getting this certification.
  • The certification process will identify qualified practitioners of transthoracic echocardiography and should enhance their professional image.
  • European Certification is designed to test the competency of an individual's ability to perform, interpret, and report routine transesophageal echocardiographic studies unsupervised.
  • Certification in TOE is not a compulsory or regulatory certificate of competence or excellence. Individuals' rights to report and sign clinical studies in individual countries remain to be defined by National laws and regulations.
  • Sonographers (technicians) and doctors of all disciplines can apply for certification in TOE.

Certification process

Obtain your certificate in 2 steps:

  1. First Step: Theoretical part - Delivered on Ipads: Candidates will have to pass this section successfully before going to the second part.
  2. Second Step: Practical part (eLogbook): Candidates must submit a logbook of 125 clinical cases (or 75 if he/she holds a valid TOE certification).

To obtain a certification, candidates must succeed both the above steps.

Theoretical: Examination on Ipad

An examination will be held twice a year in:

  • May/June - during the EACTA meeting
  • December - during the EuroEcho & other Imaging Modalities Congress

This examination is delivered on Ipads, it is a Multiple Choice Questions form (MCQ, select the best answer out of 5 possible choices) divided into two sections:

  • Reporting: 50 questions related to TOE cases with displayed echoes - around 90 minutes
  • Theory: 75 questions - 110 minutes

The examination is in English.

The duration of the written examination is 5:30 hours including a 20 minutes breakPlease note that the schedule can be subject to on-site modification.

To be successful, a candidate must pass both sections of the written exam (reporting and theory). The passing scores for each section are established after each exam within the TOE Sub-Committee, based on statistical performance of individual questions and on the average level of scores taken from the respective examinations.

Before the examination:

  • Candidates are required to come to the Certification registration desk at least 30 minutes before the start of the exam.
  • Candidates are requested to bring and present a photo ID document to confirm their identity.
  • Candidates are kindly asked to bring their calculator (the use of calculator applications on mobile phone is forbidden).

A final information letter will be sent approximately 1 week before the exam - you will need to contact the certification office if you have not received your information letter by this point.

During the examination

  • A 20 minutes break will be provided between each section.

Post examination

Results of the written examination are sent by e-mail two months after the exam.

Please note that there is no appeal process. The results are verified and checked by the committee. The results are final.

Re-certification process

As echocardiography skills can only be maintained by continued education and practical involvment, certification is granted for a period of 5 years. Afterwards, to remain certified, candidates will have to start the re-certification proces demonstrating their continued practice and learning in this field.

Registration requirements

NEW: Before paying, candidates have to complete a questionnaire where they must declare their supervisor and provide information on their training.

Please note that the supervisor does not need to be EACVI certified. Candidates who are looking for a supervisor can refer to the list of certified practitioners.

Important - Please note that in order to respect the capacity of the examination room, registrations are taken under the basis of first-come, first-served. We encourage online registration as early as possible to guarantee a seat.

Payment and general conditions

All registration fees are quoted in Euros (€) and include national applicable VAT. Payment in any other currency will not be accepted. 

Registration fees are based on the date of receipt. The prices indicated are only applicable if BOTH registration form/request AND payment are received before the registration deadlines, otherwise the higher registration fee will be applied.

Registrations will be acknowledged by e-mail upon receipt of payment and a valid candidate email address (make sure your MyESC profile is updated and ensure that no third party email address is linked to your registration). 

Cancellation policy & conditions

If a participant wishes to cancel his/her registration to the Certification exam he/she should email esccertification@escardio.org.

Please take careful note that the cancellation policy on refunds is as follows:

Please note that a "no show" is NOT considered as a cancellation. If you do not attend the exam without giving forewarning of your absence and without proof of good reason (unforeseen circumstances - such as death in the family, serious accident of the candidate...) no refund will be allowed.

If there are any queries concerning visa problems please contact us. Please note you are responsible for your visa applications and you must give at least 2 months preparation time to your respective embassy otherwise it is your concern if the visa is not granted in time.

Should a candidate not attend at the Exam, for whatever reasons, no further claims for reimbursement or waiving of fees can be made. Similarly no right to further claims or waiving of fees will arise should the ESC be obliged to alter the Exam proceedings.
By registering to the Exam each candidates waives his/her rights to claim for any compensation toward the ESC. The organiser cannot be held liable for any hindrance or disruption of Exam proceedings arising from political, social or economic events or any other unforeseen incidents beyond their control.
In no event will the ESC be liable for indirect damages or cost.
All candidates are urged to take personal travel insurance.
Registration of a candidate entails acceptance of these conditions.

Appeal process

Please note that there is no appeal process following written exam section of the certification process.
The results are the final decision of the Board.

Second Step: TOE eLogbook

Practical Part: eLogbook

After the successful completion of the written exam, the candidate can proceed with the Practical Assessment (eLogbook) which must be submitted up to a year after the date of sitting the examination.

Candidates have to submit an eLogbook consisting of: 
    1. Signed letters from supervisors (detailed below)
    2. 125 TOE anonymised studies (or 75 if  they hold certification in TTE) 
    3. 6 special cases with detailed original reports, images and media.

The particular media and details that must be uploaded in section 3 of the platform (access to be given once a pass result from the exam is received) depend on what kind of lab you work in:

What is an eLogbook?

After the successful completion of the written exam, the candidate can proceed with the  (eLogbook) which must be submitted up to 12 months after the date candidates receive their results of the examination. 
Successful exam candidates will be informed on how to submit their eLogbook along with their results. They will be sent a link and details on which username and password to use.

Please note: Cases can be collected from 1 year before to 1 year after the written exam and can cover the full 24 months.

The eLobgook can be submitted in one of the available languages (please refer to the list below).  All cases will be sent to 3 EACVI graders for review.

* Submitted reports should be the original clinical ones, but these may be accompanied by English translations which may speed up the grading process
Do remember to select the correct language when submitting your logbook. For example, if most of the logbook is in English but the reports are in Italian please submit the language as "Italian".

Documents to submit with the eLogbook:

Please note:  Any document with the candidate's name will be rejected - ensure that ONLY the candidate's number appears on the document. 

  • Signed supervisor letter documenting and reviewing the training of studies undertaken by the candidate.  
  • 125 case-summary sheet  assessed and signed by candidate's supervisor. Candidates are not asked to submit the full reports for these particular cases - although may be requested by the EACVI Graders and/or EACVI Certification Committee. Thus candidates are asked to keep this information available.

If you are having problems with the pdf template please create an excel with exactly the same column titles and use this instead. Don't forget to still include the first page singed by your supervisor

Please note that the eLogbook platform works optimally under the following conditions:

  • Use Internet Explorer as your internet browser as the platform is a Microsoft tool and can be sensitive to other browsers.
  • Please ensure that you are not logged onto any other ESC platform at the time of attempting to gain access.
  • Please ensure that all other internet tabs are closed as this can prevent access.
  • Please check that your Firewall will allow access to the platform. You may have to disable it temporarily.
  • Ensure that the server is fast enough to support uploading of all your media files
  • The platform is not supported by tablet or mobile devices.

Report requirements

These should be the original reports that the candidate has written in their place of practice. Please ensure that:

  • No patient name should be visible in the reports
  • No candidate name should be visible
  • There is a systematic description of the chambers, valves and extra-cardiac structures
  • Findings in the report (for example the severity of valve disease, chamber size or ventricular function) must by supported by relevant measurements and Doppler derived data.
  • A conclusion or summary should be included

Important:
Reports are judged on completeness and accuracy (i.e. if the report matches the uploaded images, and if there are any errors of interpretation or omission). Findings in the report must by supported by relevant measurements and Doppler derived data.

Candidates will need to submit their full, original reports for the 6 particular media cases included in section 3 of the logbook platform.

Extension requests

If a candidate is in need of an extension for their logbook submission date they must request this with a signed letter of explanation from their supervisor. The request must be submitted BEFORE the deadline of their logbook submission and cannot be granted in arrears. All requests should be directed to the certification office by email: esccertification@escardio.org.

Please note that reasons for extentions must be reasonable and cannot be granted for instances such as poor organisation and so on.

Results

Results of the complete certification process are sent by e-mail within 3 months of submitting the eLogbook.

Failure to comply with any of the above requirements will require the candidate to start the whole process from the beginning.

Appeals

Candidates who fail the full certification process (logbook section after having passed the exam) may raise an appeal to the EACVI Certification Heart House Team. Please contact us through esccertification@escardio.org

The appeal procedure for re-evaluation of a logbook is charged at 100 Euros and does not guarantee a pass after this second review. The decision of the appeal is made by the committee and is final.

Re-certification programme

EACVI Certification in TOE is valid 5 years.

Certified individuals must show evidence of continuing activity in the field.Thus in order to keep their certification status, all persons who have been certified by the European Association of Echocardiography must apply for re-certification before the 5-year validity expires.
